IINÁ PT - Pricing Fee
IINÁ Physical Therapy (PT) is a cash-based service, does not utilize insurance for payment, and is not in network with any insurance company. Cash-based payment optimizes your time and allows the licensed physical therapist to focus on your needs, not limited by insurance time or treatment restraints. PATIENT CARE
FREE! 15 minute consultation
$150 - Initial PT Evaluation - in person and online - 60-75 minutes
$125 - Follow-up PT visits - in person and online - 60 minutes
$400 - 4 visit PT package (after initial PT evaluation) - in person and online - 60 minutes per session
Dry Needling (available soon) - in person only - $30 starting cost on top of PT sessions (reduced rate). Individual session for DN 60 minutes $70.
Online Coaching - same price as PT sessions above.
*Dr. Lynnette C. Gilmore, PT, DPT is licensed in the State of Arizona, permitting physical therapy evaluations and treatment within the State of Arizona for in-person and telehealth care. Exceptions for outside of Arizona are the following: 1) Federally Recognized Tribes - please contact to discuss needs, 2) Online coaching - this is not a skilled service but can provide wellness and health information.
Medicare beneficiaries & ABN (Advance Beneficiary Notice) - Cash-based physical therapy can only provide physical therapy services that are not covered by Medicare and considered “not medically necessary”. IINÁ PT will only provide non-Medicare covered services to Medicare beneficiaries which include health, fitness, and wellness programs. An ABN form will be provided and must be signed before services to clarify payment.
Acceptable forms of payment - debit card, credit card, HSA/FSA (contact your HSA/FSA office for specific questions), cash (although card payments are preferred, many individuals do not carry wads of cash, but if you do, okay!) - no checks please.
Payment is due at the time of service.
